Illinois Insurance Code 215 ILCS 5/356m requires group health plans of 25 or more employees to cover IVF and related fertility services including up to four egg-retrieval cycles. The mandate covers medically necessary fertility preservation, including egg freezing for iatrogenic infertility.[35]

Self-funded ERISA plans are not subject to state mandate; Illinois patients on self-funded employer plans should verify coverage directly. Fully insured large-group plans are subject to the mandate. The Illinois Department of Insurance enforces.
