State insurance mandate

MGL Chapter 175 §47H requires all individual and group health insurers in Massachusetts to cover medically indicated infertility diagnosis and treatment, including IVF and fertility preservation, with no lifetime cycle cap. The mandate has been in place since 1987 and is the most comprehensive in the United States.[34]

Massachusetts MassHealth (Medicaid) coverage of fertility services is more limited; commercial insurance is the primary mandate vehicle. Self-funded ERISA plans are not subject to state mandate but most large Massachusetts employers voluntarily include fertility coverage given the strong norm.
